* In the event of changes in the information of the above-mentioned importer’s registration card, a “request to register importer” form must be enclosed – Appendix A, in addition to the relevant documents.

* Invalid importer registration card.

 

If the matter is only related a third-party warehouse, you must enclose a certificate from the district Health Office stating no objection to storing imported goods in this warehouse.
